Help Your Seniors Take Charge of Diabetes
Better Choices, Better Health-Dallas provides diabetes
self-management classes for seniors with diabetes. This Chronic
Disease Self-Management Program (CDSMP) was developed by Stanford
University. The evidence-based curriculum has become the standard
for diabetes education in 70 countries. Now, it’s available for your
patients in Dallas County.
Better Choices, Better Health-Dallas® is part of The National
Council on Aging Better Choices, Better Health®, and is the first
community-based program in Texas accredited by the American
Association of Diabetes Educators.
The seven-week, two- hour per week classes are offered to
senior Medicare participants. A medical authorization form is
required.
